Summary: grow obj_tab and dest_names arrays dynamically

Initial Comment:
experimental patch to grow the obj_tab and dest_names
arrays dynamically; arrays initially have 1000 entries.
This required also update of file avlstuff.c.

Similar to patch-366, which did this already for the
pdf_mem array. Then all (?) pdftex-specific memory
configuration parameters, obj_tab_size,
dest_names_size, and pdf_mem_size in texmf.cnf would be
ignored.
