[NTG-context] footnotes missing in heads

not so much a bug as an uncatched case (the way to deal with deeply 
nested inserts is different in lmtx than in mkiv)

(btw, it's not the latest ... that bit already changed a while ago)

(there are probably more cases that need checking)
