a quick reply:

    %~ \setuphead[section][margin=-1cm]
    \setuphead[section][alternative=inmargin]
    \starttext
    \startsection[title=Synopsis]
    \input zapf
    \stopsection
    \stoptext

Doesn’t any option suit your needs?
