Initial Comment: Put rules with their positioning on the page inside a qQ group. Should make \pdfliteral page {...} behave consistently after text or rules or images, as then any positioning starts from the PDF origin; no delta-positioning anymore. It's already being tested in pdfluatex. Just to have it at sarovar... Testing welcome, particularily with text and graphics transforms by graphicx package, to spot backward incompatibilities, if any...
