Why is my custom bullet point not rendering?

You can separate the definition from the setup. Try \defineitemgroup[prompt] \setupitemgroup[symbol=bighand,style=italic] /Mikael

The symbol is set before the value of the symbol-key is available to the environment and therefore never used.
