Can you please try if the attached patch works OK with MK IV? (You need the latest ConTeXt and you may delete some commented lines at the beginning of t-lilypond.tex. And perhaps one needs some more comments about the patch if it works.)

I've run a couple of quick tests and the patch works perfectly so far.
