how about just using a layer ...
You might also try pgf/tikz, according to your taste:
\usemodule[tikz] \starttikzpicture[scale=xxxx] \pgftext{\externalfigure[kuh][width=1cm]}; \draw (x,y) node {label}; \stoptikzpicture
