Below are four different ways I've tried to get endnotes to work. When I compile with "texexec" it work fine --- I get endnotes. However, when I compile with "texexec --lua" the footnotes are placed on the page where they are referenced. I would prefer to have the footnote/placefootnotes work so that I can easily change the output.
