A small sample file is difficult because I had the impression it did not always happen; with me these things seem to happen mostly with big files :-) Another complication is that sometimes a footnote was withheld and I could not find out what caused it. When I retyped "sentence.\footnote" sometimes the footnote did appear as it should. I sent the problem to this newsgroup just in case it was a readily recognized phenomenon. It seems not, so I propose to let it go for the moment. I will watch out for it to happen and then will first try to nail it down to a small sample before reporting again. yours sincerely, dr.
