If you can't get a minimal example, the problem is likely related to some kind of floating insert that is still dangling (like a footnote or a table or figure) or you are working in columns (the column routines have been known to be unreliable at times).
It helps to know the version of context you are using: if it is not the latest, updating context could well hand you a fix.
Otherwise, a minimal (or not so minimal) test file is definitely needed.
