A title module was recently added to the context distribution. It might be that it's interfering with your personal one. You can run your document with
\enabletrackers [resolvers.details]
and check if your title module is actually loaded. If you're sure that \usemodule is broken please post a minimal example.
It can be a problem with the file extension because in MkIV files with a mkvi or mkiv extension are preferred to files with a tex extension. For local/private modules one can use the p-prefix instead of the t-prefix because files with a p-prefix are used first.
